Application Information

If you are interested in having your child attend EFP, please complete the Application form below to begin the process. Once you have completed the form below and scheduled a time to tour our facility, you will then be able to secure placement for your child {based on availability}. Once your child has been placed in a class you will receive the required enrollment forms to complete. At that time, we will also ask for a $185 non-refundable enrollment fee. PLEASE NOTE: Filling out this form does not guarantee your child a spot as class sizes are limited. ​​All Full Day and Half Day Programs will be 12 months. (August to July) See office for more information.
